Application Form: Micro Grants for Collection/Reuse

The Micro Grant Program for Collection/Reuse is administered through Carpet America Recovery Effort (CARE) for the California Carpet Stewardship Program. The Procedures and Requirements describe project and reporting requirements, report due dates, report contents, grant payment conditions, eligible and ineligible project costs, project completion and closeout procedures, records and audit requirements. Please refer to the document titled "EXHIBIT B PROCEDURES AND REQUIREMENTS" for more information.

Section III. Project Information
All grant expenditures must be for activities, products and costs specifically included in the approved Work Plan and approved Budget. Eligible costs must be incurred, services provided, and goods received after receiving a Notice to Proceed (NTP) and before the end of the Grant Term. Eligible costs are limited to Equipment / infrastructure, Program education and outreach materials, Minor improvements to existing collections or reuse facilities or Program education and outreach materials.

Section V. Project Narrative (Concept Proposal)
Project Description*
Provide a two-to-three paragraph description of the project that explains the goals and general objective(s). Include supporting narrative that speaks to the business need, problem(s) to be addressed, potential challenges, resource estimates, and other relevant information, including product development related impacts. Note: If any of Grantee’s materials constitute trade secrets or confidential information, such materials should be clearly designated as such. Objectives identified in this section must be listed out in Section V. Project Work Plan.
